Who can refer to Jowocare?

We welcome referrals from a wide range of professional partners across London.

Social Workers

Person-centred supported living for adults across Greater London, founded on social work principles.

Local Authority Commissioners

Framework and spot-purchase arrangements available. Statement of Purpose and Capability Statement available on request.

NHS / Discharge Teams

Step-down from hospital or inpatient. We work at pace with discharge coordinators and ICB partners.

Care Coordinators / Case Managers

For clients transitioning from children's services, leaving care, or step-down from residential.

Who we can support

A brief guide to help you assess whether Jowocare is the right placement for your client.

Suitable referrals include:

  • Adults 18+ needing structured supported living
  • Transitions from children's services or leaving care
  • Step-down from hospital or inpatient settings
  • Adults with mental health needs, learning disability, or autism requiring consistency
  • Adults needing tenancy sustainment support
  • Adults on Housing Benefit (LA-funded or self-funded)
  • Adults with Care Act-assessed needs
  • Adults requiring low-to-medium support (non-personal care)

We may need further discussion where:

  • Personal care needs are identified (we are not a personal care provider)
  • Highly complex clinical needs require specialist clinical environments
  • Funding is not yet confirmed at point of referral
  • Urgent placements are requested with very limited background information
  • Current risk level may not be safely managed in supported living

What happens after you refer

A clear, structured process from referral through to decision — with a named contact at every stage.

Acknowledgement
(1 working day)

We confirm receipt and assign a named contact for your referral.

Information Gathering
(2–3 days)

We review your referral pack and may request additional information: care history, risk assessment, funding confirmation, housing benefit status.

Pre-Assessment
(within 5 working days)

A qualified assessor conducts a pre-assessment — in person where possible. We share our Pre-Assessment Summary with you.

Decision & Next Steps

We confirm suitability and, if proceeding, begin property identification and agreement planning.

Important: Housing Benefit and funding

Jowocare operates as a specified accommodation provider. Our tenants claim Housing Benefit separately through the Local Authority — not through Universal Credit housing element. Tenancy agreements are always legally separate from support agreements, in line with DWP requirements. We confirm HB eligibility before any move-in takes place. If you are unsure about funding arrangements, please contact us — we can guide you through the process.

  • Presenting needs
  • Funding source (LA/self-funded/HB)
  • Preferred borough or area
  • Urgency and timeline
  • Any known risks

Documents we will request:

  • Pre-assessment report or care history
  • Risk assessment
  • Funding confirmation letter
  • Housing benefit decision or pending application details
