We provide structured, referral-led supported living for adults 18+ across London — working in close partnership with local authorities, NHS teams, and commissioners.

Jowocare is a non-personal care supported living provider. We do not provide registered nursing or personal care — but we do provide high-quality, structured support that helps adults live independently in their own homes.

Service 01

Supported Living

Our core offer. We provide safe, stable, structured supported living for adults 18+ who need a consistent environment to maintain their wellbeing, build independence, and work toward their goals. Each placement is underpinned by a personalised support plan, developed with the individual and reviewed regularly.

  • Adults 18+ with assessed Care Act needs
  • Adults with mental health needs, learning disability, or autism
  • Adults requiring tenancy sustainment support

Service 02

Transitions Support

We specialise in supporting young people and adults moving from children’s services into adult provision — one of the most vulnerable transitions in the social care system. We work closely with leaving care teams and children’s social workers to ensure the move is planned, supported, and sustainable.

  • Care leavers aged 18–25
  • Young adults transitioning from children’s services

Service 03

Hospital Discharge Pathways

We work at pace with NHS discharge coordinators, ICB partners, and social workers to provide rapid step-down placements for adults leaving inpatient settings. We understand the pressures of discharge pathways and respond quickly without compromising on quality.

  • Adults stepping down from hospital inpatient
  • Adults leaving mental health wards
  • Adults exiting residential rehab

Service 04

Mental Health & Learning Disability Support

Consistency is everything for adults with mental health needs, learning disability, or autism. We provide stable placements with keyworkers who understand the importance of routine, rapport, and person-centred planning. We do not provide clinical or personal care, but we do provide an environment where people can flourish.

Service 05

Tenancy Sustainment

Many of the people we support have had disrupted housing histories. Tenancy sustainment is woven into everything we do — from practical support with bills and routines to advocacy with Housing Benefit and the local authority. We want people to stay, thrive, and eventually move on when they’re ready.

  • Adults at risk of tenancy breakdown
  • Adults with a history of homelessness or housing instability
  • Adults requiring support to manage independent living

Partnership Working & Commissioning

We welcome formal partnership arrangements with local authorities and commissioners. We can operate on a spot-purchase or framework basis and are happy to discuss bespoke arrangements for specific client groups.
