Jowocare was founded by a qualified social worker who has worked inside the system — and experienced first-hand what happens when support isn't properly coordinated. We exist to close that gap.

We're not a large agency. We're a focused, values-driven organisation that takes quality of support seriously — focused entirely on adult supported living (not personal care). That means the people who work here matter enormously, and are treated that way.

Core Role

Support Worker

Working directly with individuals in their own homes, helping them build daily living skills, access their community and work towards their personal goals.

  • Flexible hours — part-time and full-time considered
  • 1:1 support in the community and at home

Specialist Role

Keyworker / Named Worker

Taking a lead role with a small caseload of individuals — being their primary point of contact, leading reviews and coordinating the support around them.

  • Experience in supported living or social care required
  • Strong coordination and communication skills
  • Involvement in support planning and reviews
  • Liaison with commissioners and multi-agency professionals

Freelance Practitioner

For experienced practitioners who want to work flexibly alongside Jowocare on a self-employed basis — providing support, assessments or specialist input as needed.

  • Social workers, OTs, mental health practitioners welcome
  • Flexible, project-based arrangements
  • Suitable for those working across multiple providers
  • Must hold own professional registration and insurance

Transitions Specialist

Transitions Support Worker

Specialist role working with young adults (18+) moving from children's services into adult provision — including those with EHCPs, care leavers and young people leaving semi-independent settings.

  • Experience working with young people in transition
  • Knowledge of EHCPs, Children Act and Care Act
  • Relationship-building and advocacy skills
  • Liaison with leaving care teams and SEND professionals

NHS Pathway

Hospital Discharge Support Worker

Supporting individuals stepping down from hospital or inpatient settings — providing structured, community-based support that reduces the risk of re-admission.

  • Experience in NHS, mental health or discharge settings preferred
  • Ability to work at pace with referrals from hospital teams
  • Strong risk awareness and lone working competence
  • Understanding of D2A and step-down pathways

Sessional Cover Worker

Providing flexible, sessional cover across our placements when regular workers are unavailable — ideal for those wanting occasional work alongside other commitments.

  • Ad-hoc availability required
  • Experience in care or support essential

Skills & experience

We value attitude and values as much as qualifications. If you're committed to supporting people with genuine care and professionalism, we want to hear from you.

Essential for all roles:

  • Enhanced DBS certificate (or willingness to obtain one)
  • Right to work in the UK
  • Commitment to safeguarding and person-centred values
  • Good communication and record-keeping skills
  • Ability to work independently and as part of a wider team

Desirable:

  • NVQ Level 2/3 in Health and Social Care (or working towards)
  • Experience in supported living, mental health or learning disability support
  • Knowledge of the Care Act, Mental Capacity Act or Children Act
  • Experience with EHCP, transitions or leaving care pathways
  • London driving licence (helpful but not essential)
